19 yr old college basketball player with transverse patellar fx OCT 96, treated with cannulated 4.5 mm screw. Refracture playing B-ball in Jan 97 with the cannulated screw ripping through the proximal fragment. Fracture fixated with 2 transverse 3.5 mm screws and 2mm steinman pins with 18G tension band wiring. At 4 months post-op, pt without extensor lag, and motion 0-120. X-rays show 1.5 mm gap at the articular surface and 4mm gap at the ventral cortex. There is no step-off of the articular surface.
