The bridge would make me to try closed management - gradual alignment
and compression by the external fixator while lengthening is being
performed at other levels.
Of course some sort of bone grafting also can be performed, either
open or closed. The latter can be preferred since anyway 6-8 weeks
would be spent in ex-fix to obtain needed length. I mean perQ
osteotomy of the anterior part of the proximal fragment and its
transport over the gap.
