You can reduce the wing first then the anterior column. The post column can be reduced thru lateral and middle windows with an angled jaw clamp (matta clamp). The stoppa window may allow you to just use the lateral window of the ilioinguinal approach. Although I agree that the anterior column should be reduced, what about posterior column displacement and conminution? I can't imagine addressing the lateral fragment of the dome from ilioinguinal. Are you telling me that the anterior approach shoud be done first and in a second operation I should do the posterior approach or that there is a chance that the anterior approach will be enough, because the posterior elements will reduce as I reduce the anterior column?
