[ORT-L] peri-prosthetic femur fracture

[ORT-L] peri-prosthetic femur fracture Can't Post

This 44 yo female had a THA 2/07. 4/07 she slipped and sustained a spiral fracure which I treated, obviously not indirectly. This was very difficult to reduce (long limb due to THA?). There was a split in the proximal femur (hence the cable). Has been kept PWB. Fatigue fracture of plate this weekend.

I am planning repeat plating. Probably going to use Infuse, though I would consider autogenous graft as well. I am thinking about femoral allograft strut afixed to the medial side with cable proximal, screws below. Any advice ?

In a similar case i presented here for advice we decided to give up plating
and turn to nailing as more biomechanically beneficial, and because of
cylindrical shape of the stem tip (DePuy AML). A custom-made nail was used.
The surgery was 4 weeks ago. Now she walks with a cane. Looks like the
distal stem in your case could allow something like this.
