Dear members

Just a straw poll of opinion

Would you accept non-operative management (no further manipulation) of a distal radius greenstick fracture (metaphyseal) of say 10 degrees VOLAR tilt in a 6 years old?

How about 13 years old?

What if the fracture is complete?

Yes in both age groups 10 degrees would be acceptable. In greater degrees
of angulation gender and maturity need to be considered as well.

summarizes and references the consensus of major textbooks and studies
regarding acceptable limits of angulation etc in orthopaedics.

Just thinking about the throckmorton sign. Just kidding, I saw your old email while "cleaning up a bit." How is life back in the south. NYC is pretty good. cool, I like straw polls.

1) absolutely in a 6 y.o., apex dorsal or volar angulation no
difference in whether I'd accept
(immobilization in neutral, pron, sup is debated)

2) with hesitation, if a boy maybe, girl no
(differences on when skeletal maturity occurs therefore
ending remodeling ability)

3) no difference complete or greenstick/torus, as long as not too
short or excess varus/valgus instability/DRUJ incongruence if complete

Actually I do have that book ( I love bite-sized books, pity I can't get in electronic format)

I believe 15 degree quoted for the older age group in your book.

I worked with (or for) some with an absolute distaste for any volar tilt. Sure, it may take a bit longer to remodel but I cannot believe that 15 degrees can result in clinical deformity....

Maybe one can make a claim that cast for minor volar tilt complete fracture is harder to mould but a greenstick is as stable as they come.

Just thought I'd check in case I am out of touch.....
