Group, Will be interested to have your experience on this. We had a patient involved in a high velocity road accident and brought to hospital in shock within 20 minutes. Resuscitated with fluids, blood, mechanical ventilation etc. Required inotropic support for a short time. Total blood transfusion was over 20 units. The injuries included pelvi-acetabular fractures, liver tear, chest injuries etc. Managed non-operatively due to the extremely unstable haemo-dynamic status secondary to the injuries. Patient later developed pulmonary embolism and had to be put on therapeutic anti-coagulation. Now almost 5 months down the line and a Doppler study of the lower limb suggest features of AV-fistula. Now awaiting further vascular studies. Has anyone encountered A-V fistula formation following pelvi-acetabular fractures? Any reports of the same? Dr. T. I. George.
