A 74-year-old female presented with a 3-month history of compression neuropathy of the right ulnar nerve in Guyon's canal. Magnetic resonance imaging and ultrasonography revealed the location of the mass lesion. Surgical exploration discovered a lipoma pressing against both the ulnar nerve and the ulnar artery. This is the most common benign soft tissue tumor of adults. They are sometimes multiple and usually painless and asymptomatic. Malignant transformation is virtually never seen. Angiolipoma is a common variant which has a proliferation of small vessels scattered throughout the fat. Unlike common lipomas, these tumors are usually painful.
