Description: Radiology case 454-412 Clinical presentation: Fall by boy aged 8, now pain in the leg.
The view is of the proximal metaphysis of the tibia in an immature skeleton. The bone cortex is intact, but shows a slight irregularity medially, marking the end of a transverse line of irregularity of trabecular detail. The trabecula do not join-up properly. The appearance is due to buckling of the bone with minute fractures of the medullary, that is cancellous, bone. If seen obliquely, the dense line resembles a doughnut, which gives the fracture its name. The epiphysis and epiphyseal plate appeared normal.
